An Always Family Senior Care Advisor will meet with your family to help determine if home care is the most practical option for the recovering patient. The Advisor will take the time to answer all of your questions, provide information about our comprehensive transition care program, and put you in touch with one of our RN Care Coordinators to help facilitate the care process.

Personalized health care plan
Our caregivers operate as an extension of your loved one’s primary care team. Your RN Care Coordinator will communicate, collaborate, and coordinate with doctors, therapists, social workers, and patient families to develop and execute a complete healthcare plan tailored to your loved one’s needs. Everyone is kept updated on the recovery progress and will make recommendations on tweaking the care plan when needed.
Personal Care
Our caregivers will assist your loved one with personal care tasks, including bathing, grooming, dressing, toileting, and more so the person feels clean and comfortable. We carry out these tasks with compassion, patience, and respect, making special efforts to maintain your loved one’s dignity at all times.
Light housekeeping
We want your loved one to focus on recovery in a clean, safe, and clutter-free space. Our team will help take on light housekeeping duties such as laundry, dishes, floor cleaning, tidying up, and more. Our goal is to help preserve a fresh, stress-free environment for your loved one.
Meal preparation
Proper nutrition is a crucial component of any care plan. Our caregivers can help by planning wholesome meals, shopping for ingredients, and preparing them in your loved one’s kitchen. We cater meal plans to virtually any special dietary needs and every nutritional, allergic, or religious requirement.
Transportation and accompaniment
When your loved one needs to attend medical appointments or get to other destinations, we can help with our reliable transportation and accompaniment services. Our team will ensure that your special person makes it to each destination on time and returns safely home.
Medication reminders
Taking the correct dose of medications at the right time is crucial to your loved one’s recovery and long-term health. We’ll help manage the person’s medication to ensure the person takes all prescription and non-prescription medications as recommended by the doctor or pharmacist.
Alzheimer’s disease and dementia care
Our caregivers have all completed extensive, specialized training to support older adults with progressive cognitive conditions. We’ll introduce therapies that help slow memory loss and employ gentle, reassuring tactics to keep your loved one calm and safe.
